Executive Vice President, Sanford Health and President/CEO, Sanford Health Plan

Tommy Ibrahim, MD, MBA, MHA, is executive vice president and chief transformation officer for Sanford Health, the largest rural health system in the country. Headquartered in Sioux Falls, SD, Sanford Health is an integrated, nonprofit system serving more than 2 million patients across the upper rural Midwest, with a network that includes 58 hospitals, 289 clinic locations, 145 senior care communities, 4,500 physicians and advanced practice providers and 1,100 active clinical trials and studies.

In this role, Dr. Ibrahim oversees Technology, Digital, AI Strategy, Data Analytics, Innovation and Sanford Research, and he partners with system leaders on the transition to value-based care. He is focused on driving Sanford Health’s transformation strategy and roadmap; ensuring strong AI and data governance; accelerating progress responsibly and demonstrating measurable impact in employee and clinician satisfaction, patient experience, affordability, operational and financial performance.

Under Dr. Ibrahim’s leadership, Sanford Health is scaling AI-enabled end-to-end operational reimaginations and proactive care models that are delivering measurable improvements in early detection and intervention. These include a chronic kidney disease tool that has doubled testing and tripled early diagnoses among patients with diabetes, and a colorectal cancer risk tool embedded in the EMR that analyzes 85 factors to prioritize screenings — helping close care gaps and improve outcomes for patients across rural geographies.

Dr. Ibrahim also leads a rural-focused research enterprise engaging more than 21,000 participants in Sanford-led clinical trials and studies — one of the largest rural-focused research cohorts in the region — with a commitment to building the foundation for future treatments driving evidence-based care.

Dr. Ibrahim is a nationally recognized leader in rural health care transformation and a sought-after voice on the future of care delivery. He regularly speaks on the role of innovation, AI, technology, patient experience, affordability and value-based care in shaping the next era of rural health.

He also serves in several leadership and advisory roles across the health innovation ecosystem, including advising select portfolio companies for Abundant Venture Partners, a16z and Redesign Health.

Prior to this role, Dr. Ibrahim served as president and CEO of Sanford Health Plan, which serves 415,000 members across South Dakota, Wisconsin, Minnesota, Iowa, Nebraska and North Dakota. Before joining Sanford Health, Dr. Ibrahim served as president and CEO of Bassett Healthcare Network and has held numerous leadership positions within the health care industry for more than 15 years.

Before joining Bassett in 2020, he served as executive vice president and chief physician executive for INTEGRIS Health in Oklahoma, as chief physician officer and vice president of medical affairs at MercyOne in Des Moines, Iowa and as senior vice president and chief physician executive at St. John’s Hospital in Springfield, Illinois.

A board-certified hospitalist, Dr. Ibrahim received his Doctor of Medicine degree from St. Christopher’s IMD College of Medicine and completed his internal medicine residency at Greater Baltimore Medical Center, an academic affiliate of Johns Hopkins, and is licensed in multiple states. He holds a Master of Business Administration degree from Columbia University, a Master of Healthcare Administration degree from Seton Hall University, and is a Fellow of the American College of Healthcare Executives and a Fellow in Hospital Medicine.

Dr. Ibrahim and his wife, Marian, have two children and reside in Sioux Falls, SD.
